Homeless Outreach Support Team (HOST)

Bridgeway's Homeless Outreach Support Team (HOST) program, serves homeless individuals of all ages living with serious mental illness, serious emotional disturbance, and/or co-occurring substance use disorders (COD). We prioritize the enrollment of transition age youth (TAY), ages 16-24, as well as those with co-occurring substance use disorders. The HOST program provides accessible, effective, coordinated, integrated, and evidence-based treatment services, peer support, and other recovery support services, and linkages to sustainable permanent housing to individuals living with BH conditions. HOST counselors work to achieve holistic outcomes related to health/BH, housing stability, and wellness for individuals served.

HOST services include:

  • Hands-on assistance with finding both temporary and permanent housing.
  • Linkages to mental health, housing co-occurring (substance abuse) service, medical, legal, education, career development, transportation and social services.
  • Assistance with obtaining public assistance and/or social security benefits, if desired.
  • Individualized recovery plan to assist with reaching goals.
  • Support in living in the community.
  • Assistance with daily living skills, including budgeting, personal hygiene, housekeeping, and medication education.
  • Transportation to initial appointments, or when necessary.

GENERAL HOURS

The HOST services are open seven days a week, 9 am to 8 pm, and are on call after hours.

Bridgeway’s HOST program is located at 615 North Broad Street, Elizabeth, as a HOST Hub where homeless individuals in our target population, including TAY and individuals with COD, can drop in to make use of resources and/or receive care management.
