Homeless Outreach

Bridgeway provides outreach, support, and case management services to adult Union County residents who are homeless, or are at risk of becoming homeless and who have a mental illness.

Program of Assertive Community Treatment (PACT)

PACT provides comprehensive, integrated rehabilitation treatment and support services to those individuals who are most challenged with their serious mental illness. PACT believes that people who have serious mental illness can live successful and satisfying lives in the community when flexible services and supports are provided.

Residential Intensive Support Team (RIST)

RIST is an intensive residential support program designed to address the needs of individuals who may require intensive but varying degrees of residential support in the transition from hospital to community living. Bridgeway's RIST programs are comprehensive, flexible, mobile teams skilled in state of the art interventions that provides person centered recovery and wellness services to people re-entering the community from a psychiatric hospital. RIST, as a housing model, supports individuals' use of other community mental health treatment, employment and rehabilitation services, as appropriate.

Supportive Housing & Enhanced Supportive Housing

Bridgeway’s Supportive Housing program assists people with mental illness who want to live independently. Supportive housing helps persons receiving services to gain access to affordable, permanent housing, link them to employment and educational programs and with community supports, and decrease their hospitalization rate. The strategy of matching affordable housing with home/community-based services is proving very effective. Addressing housing first results in positive outcomes for career development, community integration and re-hospitalization reduction. Improved outcomes in the future will be realized as people adjust to new living environments, become more focused on goals for the future and staff is able to devote time to work with them on their on rehabilitation goals.
